Social Worker (Uptown Goddard) Job Details Job Location MOC: Outreach Uptown Team - New York, NY Salary Range $56,000.00 - $61,000.00 Salary/year Description Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. Program Description:The Manhattan Outreach Consortium is a borough-wide initiative to reach out to and quickly move street homeless individuals into permanent housing. The Uptown team serves the UWS and UES of Manhattan and collaborates with various community partners and healthcare agencies throughout Manhattan. The catchment area includes W. 59th - W. 110th Street & E. 59th - E. 96th Street, river to river. Purpose of Position: To coordinate and support the activities of the team's medical and psychiatric providers to deliver integrated care, be an additional support in completing clinical assessments and potential emergency removals via 9.58 of clients. Schedule:Monday through Friday 9:00 am to 5:00 pm Salary Range:$56,000-$61,000 Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account. Roles, Responsibilities, and Essential Duties Manage psychiatric and Street Medicine Team (SMT) schedule: Add clients to the schedule when referral documents are provided Sign in and sign out with treatment team members on their scheduled office days Ensure Housing Outreach Specialists submit referrals to SMT/psych and facilitate follow up on recommendations from treatment team Develop linkages with other healthcare providers and resolve insurance issues: Gather medical information received from other appointments or hospital visits Assist Housing Outreach Specialists with obtaining medical records from hospitals Find providers (primary care, specialty, additional health care services) for clients based on provider recommendations, geography, and insurance coverage. Assist with insurance authorization process Consult E-PACES to determine client medical coverage when necessary Track clients' Medicaid/Medicare certification to ensure recertification Communicate: With hospitals and emergency rooms during client acute treatment/hospitalization With Janian providers about emergencies, need for advocacy, other urgent matters Field work: Canvass for clients in the field for psych/medical appointments (either with SMT/psych providers or independently) Escort clients to medical appointments, labs and diagnostic imaging centers, as needed Meet monthly with team leadership and SMT providers to review and update medically vulnerable list Carry a caseload of the teams' more entrenched, psychiatrically vulnerable clients Obtain 9.58 designation and aid in assessing and completing removals for clients in the field when needed Assist SMT with coordination of special projects and public health initiatives such as vaccinations Lead in clinical discussions in team meetings Other duties as required Qualifications/Educational Requirements MSW required at hire, LMSW required within first 3 months of hire Valid driver's license and a driving record that meets the agency's auto insurance guidelines (including no suspensions within the last 3 years) Flexibility in work hours; available to work evening, overnight and weekend shifts when required (participation in 4 overnight counts a year) Skills, Knowledge and Abilities Ability to multi-task/work in a fast-paced environment Proficiency in Spanish and English preferred Understanding of housing first and harm reduction models Experience with homelessness preferred Strong organizational and communication skills Ability to work as part of a team Computer Skills: To perform this job successfully, an individual should be: Proficient in Microsoft Office: Word, Excel, Outlook Able to use or learn computer databases such as MOCapp, CARES, E-PACES Physical Requirements Frequent walking and standing Occasional lifting of up to 30 lbs Work Environment Indoors and outdoors; all weather Employee Benefits : Medical/Dental/Vision Life Insurance/Commuter benefits/Employee Assistance Program (EAP)/Flexible Spending Accounts 403B Thrift Retirement Plan 12 Annual Sick Days 12 Agency Holidays 20 Days or 4 Weeks of Paid Time off and an additional week (5) days after 5 years of employment Total of more than 6 weeks (a month and half) of paid time off We are committed to building an inclusive workplace community grounded in respect for differences.
