We are searching for a dedicated Home Infusion Pharmacist to join our dynamic healthcare team. In this role, you will play a crucial part in providing pharmaceutical care for patients receiving infusion therapy at home, ensuring safe and effective medication administration, and improving patient quality of life. The ideal candidate will possess a background in home infusion pharmacy or a willingness to learn, a commitment to patient well-being, and excellent communication skills. Key Responsibilities: Conduct thorough medication reviews and develop personalized care plans for patients receiving home infusion therapy. Prepare and dispense intravenous medications, ensuring accuracy and compliance with regulatory standards. Provide patient and caregiver education on the proper administration of infusion therapies, potential side effects, and storage of medications. Monitor patient outcomes and adjust therapy as needed to ensure optimal therapeutic results. Collaborate with healthcare providers, home health nurses, and patients to optimize medication therapy and resolve any drug-related issues. Manage inventory and procurement of infusion medications and supplies, ensuring proper storage and handling. Assist patients with navigating insurance coverage and prior authorization processes for infusion therapies. Work with insurance companies and financial assistance programs to ensure patients have access to necessary medications. Stay current with advancements in infusion therapy pharmaceuticals and techniques. Participate in quality assurance programs and continuous improvement initiatives to enhance pharmacy services. Preferred Qualifications: Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program. Experience in home infusion pharmacy. State pharmacy license in good standing. Proficiency in preparing and dispensing infusion medications. Strong analytical and problem-solving skills. Excellent communication and collaboration abilities. Certification in infusion therapy (e.g., CRNI, IgCN) preferred. Familiarity with electronic health record (EHR) systems. Dedication to ongoing professional development and continuous improvement.
