Summary of Responsibilities:Under the general direction of the Family Finding & IL Supervisor, this person will work to engage extended family members in service and involvement with the child and family. This position will perform activities on behalf and in support of the Family Finding Program including promotion of the Family Finding Program with Case Management, referred families and the community. This person will also provide case management for Resource Families. Duties are conducted in accordance with all existing policies, procedures and regulations and in accordance with the Mission and Values of the Company. This position has been designated to be a part of the County's CYS workforce for purposes exclusive to the proper handling of consumer information as is required by SAM privacy policies.Essential Functions:Maintains case recordsMeets with families to set up meetingsAssists/facilitates family meetingsPrepares documentsProvides transportation to meetingsCreates eco maps with families and youthAssists families in building their family treeAssists with required referralsDevelops plansEngages families during meetings and processesPrepares Family Finding documentationMaintains the Family Finding fileAssists with paperwork to access servicesPlans/organizes/leads family finding meetingsCollects and reports dataAttends required trainingsPrepares court documentation and testimonyEnsures referral source needs are being metCompletes contacts with Resource FamiliesRecruits actively and retains Resource FamiliesAssists in locating appropriate placements for youthMaintains resource family records, per 3700 RegsMonitors ICPC CasesPersonal Characteristics:Independent, Tech-Savvy, Responsive, Kind, Ethical, Motivated, Active Listener, Life-Long Learner, Adaptable, Confidential, Resourceful, Values, & Trustworthy Education and Experience:Bachelor's degree with completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functionsEmployment Requirements:Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening ClearanceWorking Conditions:Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift to 15 pounds.
