ProMedica Toledo Hospital is one of the largest acute-care facilities in the region. Under the direction of the Manager of Jobst Medication Therapy Management, performs ambulatory care clinical pharmacist functions necessary for the daily operations. The clinical pharmacist must be willing to cover at any Jobst Vascular Institute - Jobst Medication Therapy Management site at any time as deemed appropriate. ACCOUNTABILITIES In collaboration with providers, manages referred patients' medication therapy as outlined in the Collaborative Practice Consult Agreement. Communicates politely, effectively, and professionally with patients, other pharmacists, and all other healthcare providers verbally in-person and via phone, as well as written through mail and electronic medical record (EMR). Clarifies any medication questions with any of the patient's providers/specialists when needed. Cooperates with management and peers to promote an environment of teamwork and collaboration. Performs point-of-care tests and/or orders disease related tests based on referred patients' medication therapy; documents results and communicates results following policies and standards of ProMedica clinical laboratory. Performs outpatient chart review for appropriate medication usage and provides patient counseling about related drug therapies. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-disease interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of therapy, and therapeutic duplications. Utilizes standard work, as well as disease state and medication assessment per evidence based care plans, protocols, and clinical judgment. Using EMR, documents all communications/activities/interventions accurately and thoroughly and directs any messages as needed to the appropriate person. Using EMR sends Standing Order/Confirmation of Verbal Order/specific request to a lab, home health agency, extended-care-facility, physician, etc. Using EMR, schedules/reschedules patient appointments for the pharmacist visit and submits accurate charges for patient encounters. Contributes to education of pharmacy residents, pharmacy students, and medical students as assigned. Educates staff, prepares patient educational handouts, and reviews current materials and drug information. Participates in medication related research and projects. Participates in quality improvement activities required by the Joint Commission, ProMedica hospitals, Jobst Medication Therapy Management, Quality Review Teams or others as needed. Demonstrates compliance with licensing, regulatory and accrediting agency provisions. Complies with established departmental policies and procedures, objectives, quality assurance programs, and safety standards. Evaluates current policies for review and revision as necessary. Attends orientation, education, and training programs. Reviews literature and other materials as appropriate for the position. Attends mandatory staff meetings and performs other related duties as assigned or requested. Education: Must be a graduate of an Accredited School of Pharmacy (ACPE) and have a Doctorate of Pharmacy (PharmD). Skills: Must be able to understand directions, communicate and respond to inquiries effectively and timely - requires effective interpersonal skills. Must function as an integral member of the team. Must be able to input and retrieve information from the computer. Must be able to move about the facility and between workstations. Years of Experience: Must have completed a PGY-1 (Post Graduate Year 1) pharmacy residency (for all new hires) or at least two to three years of hospital pharmacy experience (for any previous hires). License: Must be an actively registered pharmacist (RPh) licensed in state of practice - primary source online verification required directly from State of Ohio Licensing or State of Michigan Licensing database. Must obtain other state license within 1 year of hire. Certification: Must have a certificate of registration and a current identification card in state of practice. Must maintain Basic Life Support certification. PREFERRED QUALIFICATIONS Years of Experience: Completion of a PGY-2 (Post Graduate Year 2) pharmacy residency Certification: Board of Specialties, Certified Anticoagulation Care Provider, Certified Diabetes Educator, other certifications relating to pharmacy ambulatory care WORKING CONDITIONS Personal Protective Equipment: Must wear gloves during all patient interactions. Suggested to use protective eyewear when performing point-of-care testing. Physical Demands: Weight requirements according to the Department of Labor's dictionary of occupational titles. Must be able to occasionally move, lift, and/or carry very heavy objects and materials. Must be able to visually tolerate exposure to blood.
