Check us out online: Work Shifts 7:00 A.M - 3:30 P.M 8:00 A.M - 4:00 P.M 8:30 A.M - 4:30 P.M 9:00 A.M - 5:00 P.M 10:00 A.M - 6:00 P.M Job Description Purpose of Position: Provides person-centered, strength-based assessments, evaluating barriers to access and adherence that are tailored to develop treatment/care plans focused on the individual needs and goals of the patient. The assessments will integrate health care and social support services Responsibilities: Develop an interdisciplinary, patient-centered care plan Ensure patient with multiple chronic conditions/ disabilities achieve the highest personal level of functioning Improve communication among providers/ care team, patients and their caregivers Offer a range of services for high risk patients, including psychotherapy and social support Support family care givers to reduce stress and burden Minimum Qualifications For Assignment to Level II: 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED. Salary may differ based on the licensure (LMSW or LCSW) of the candidate.
