Work Shifts 9:00 A.M - 5:00 P.M Job Description PURPOSE OF POSITION: It is essential to employ a registered dietitian (RD) in the Ambulatory Care - Women's Health Practice. The dietitian provides tailored counseling to all patients referred to ensure optimal nutrition status and future health. AREAS OF RESPONSIBILITIES: The dietitian interviews patients to obtain nutrition history, reviews medical records data, and completes nutrition assessments and reassessments according to evidence-based nutrition practice guidelines. The dietitian as well provides medical nutrition therapy to ambulatory care patients and families; consults with members of the healthcare team regarding the nutritional needs of patients; establishes and maintains an effective nutrition education program for the Women's Health Practices; participates in health care activities in the Ambulatory Care Practice and assists with the implementation of departmental policies and procedures. SUMMARY OF DUTIES AND RESPONSIBILITIES: • Interviews patients/significant others to obtain nutrition history, reviews medical records data and completes nutrition assessments and reassessments according to the Evidence Based Nutrition Practice Guidelines. • Documents in the Electronic Medical Record (EMR) in ADIME format utilizing the Nutrition Care Process. • Provides medical nutrition therapy (MNT) to ambulatory care patients and/or families and documents nutrition intervention in EMR. • Considers cultural and religious food preferences, language barriers, physical disabilities and age group when performing assessment or providing MNT. • Assists in supervision for subordinate dietitians. • Plans and directs work of subordinate staff in the absence of Clinical Nutrition Manager and Level IV dietitian. • Assists in preparation of work schedule for subordinate dietitians and supervises scheduling of work assignments, prepares schedule when requested. • Acts as a liaison among leadership and dietitians. • In the absence of Clinical Nutrition Manager and level IV dietitian, conducts staff conferences in order to communicate new and/or revised policies, procedures, and other appropriate matters. • Responsible for all aspects of patient nutrition care and education of patients and/or families referred by physician. • Consults with members of the healthcare team regarding the nutritional needs of patients. • Collaborates with the interdisciplinary team to maintain or improve patient care and outcome metrics. • Participates in the healthcare team activities in the Ambulatory Care Services, including meetings and rounds. • Establishes and maintains an effective nutrition education program for Ambulatory Care. • Maintains file on nutrition group education program and individual patients counseled. • Documents broken appointments in medical chart and send scheduled appointment date • Serves as a professional and educational resource to subordinate dietitians and other staff. • Participates in training of new clinical dietitians and dietetic interns. • Participates in special nutrition projects for patients, when requested. • Participates in hospital and community events, including developing and presenting nutrition related lectures/presentations to various community groups, when requested or assigned. • Assists in the development and presentation of education and training programs for the professional and auxiliary staff. • Participates in data collection for performance improvement. Performs and maintains performance improvement programs as assigned. • Submits monthly productivity and statistics of ambulatory nutrition visits. • Assists with the development and implementation of departmental policies and procedures. • Participates in committees, attends meetings, trainings, continuing education and in-services, as assigned. • Participates in down time procedures during EMR failure. • Maintains patient confidentiality and discards Protected Health Information (PHI) as per hospital policy. • Performs related tasks as assigned, and assists others in the department as requested by appropriate personnel. Minimum Qualifications Qualification Requirements: For appointment to Assignment Level I 1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or, 2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. For appointment to Assignment Level II 1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. For appointment to Assignment Levels III and IV 1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
