Pharmacy or PharmD Required Related Experience none Required License/Registration/Certification Licensed pharmacist or license eligible in Kentucky Physical Requirements May require standing or sitting for greater than 5 hours at a time. Shift Varies Job Summary In this position the candidate will provide pharmaceutical care to patients admitted to the University of Kentucky Healthcare Albert B. Chandler Medical Center. Responsibilities include: reviewing and perfecting medication orders, reviewing drug profiles to ensure appropriateness of therapy and contacting and conferring with physicians and other practitioners to optimize medication therapy. Actively participating in all aspects of the medication reconciliation process and promotion of economically conscientious use of medication. Daily activities may include active participation on patient care rounds, review of patient profiles, and provision of pharmacokinetic evaluation of therapeutic regimens. Skills / Knowledge / Abilities knowledge of inpatient pharmacy practice, strong clinical skills, ability to prioritize Does this position have supervisory responsibilities?
