Position Summary Under the general supervision of the Director of Pharmacy Services the Clinical Pharmacist will assume the responsibility and accountability for the management of rational antibiotic therapy used within the Hospital , or management of the critical care or high risk patients as defined by area of responsibility. Will provide for the collaboration and consultation with other health professionals on the therapeutic use of antibiotics or other medication for patients under the scope of responsibility. Will also provide education and training for health professionals involved in this clinical forum. Essential Functions and Responsibilities Includes the Following: 1. Understands and adheres to the Hospital Performance Standards, Policies and Behaviors. 2. Improve the monitoring and management of the clinical pharmacy antibiotic service or of critical care or high risk groups to include interventions to improve the quality and cost effectiveness of patient care. 3. Promotes the clinical pharmacy development of pharmacy staff by providing clinical leadership and education as appropriate. 4. Work collegially with others in all activities in order to achieve organizational objectives, cooperating in both interpersonal and team relationships to establish common ground, build shared goals and encourage new ideas. 5. Generate creative approaches and new ideas to improve performance, service and quality. This includes identifying and weighing options and interventions, involving appropriate people in the decision making process and making sound decisions after reviewing all relevant information. 6. Takes personal responsibility for all work activities and personal actions including following through on commitments, implementing decisions that have been agreed upon and acknowledging mistakes or failures without blaming others. 7. Performs all other related duties as assigned. Education & Experience Requirements Graduate from an accredited school of Pharmacy with a PharmD degree. Current New York State Registration. Retains composure under stress Physical/Mental Demands/Requirements & Work Environment Must be physically able to ambulate or be stationary for extended periods of time, perform difficult manipulative arm-hand steadiness and hand-eye coordination skills. Must be physically able to meet all regulatory requirements. Ability to focus and concentrate on fine detail with interruption and attend to tasks for more than 60 minutes at a time. Individual must be able to understand theories behind several related concepts and remember multiple tasks given during the course of a day. Must be able to detect, interpret and judge objects both closely and distant. May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital's SDS (Safety Data Sheet) data base and may be accessed through the hospital's Intranet site (Employee Tools/SDS Access). A copy of the SDS data base can also be found at the hospital switchboard, saved on a disc.
