JOB TITLE: RN Home Health visit nurse STATUS: Part time - mornings at 8am or evenings at 8pm DEPARTMENT NAME: Clinical REPORTS TO: Clinical Director and Administrator of Home Care and Home Health. EverHome HealthCare (EHHC) has been helping clients since 2007. We are a locally Doctor owned home health and home care company. Our main office is in Lynnwood, and we serve clients In Snohomish, King, and Pierce counties, helping them to live as independently and meaningfully as possible whether they are ageing or recovering from injury or illness. The Registered Nurse Case Manager (RN) is responsible for the day-to-day skilled nursing care of clients receiving services from EverHome Healthcare (EHHC). Top 4 duties and responsibilities: Provide home health visits. RN delegations to caregivers (We can train you) Admit clients to home health services by completing start of care documents, including but not limited to initial nursing assessment, home safety assessment, fall risk assessment, and plan of care. Prepare and submit clinical notes at the end of each visit. We do not take Medicare/caid clients, so the documentation is so easy. Qualifications: Must be a licensed Registered Nurse in the State of Washington. 3 years clinical experience; preferably in rehabilitation, home care, or hospice. Must have prior TPN experience or willing to be trained. Must demonstrate an ability to practice independently in the delivery of nursing care as well as be a problem solver Ability to read and write in English. Excellent written and verbal communication and presentation skills. Require reliable vehicle for use when traveling to work sites. A valid Washington State Driver's License and proof of current auto liability insurance Computer literate with experience in different computer programs Creative, flexible, and innovative.
