We are looking for a Behavioral Health Associate to work at our La Junta Clinic in La Junta, CO. The Behavioral Health Associate will work as part of an integrative, primary healthcare network, using a Team-Based Care (TBC) approach to focus on holistic care for patients, across all ages with the goal to provide preventive, therapeutical and educational services. Fostering a culture of patient and family-centered care, service excellence and safety while enhancing patient, employee and community relations. Essential duties and responsibilities: Provide clinical assessment, problem identification, need for medical necessity (DSM 5 criteria/justification), evidence based treatment modality, crisis intervention, individual/family/group therapy or other activities, case management, advocacy and outreach/prevention and interventions Collaborates with the care team, following the Primary Care Behavioral Health (PCBH) model, developing and implementing evidence-based treatment plans and interventions through consultation, brief assessment, and psychoeducation. Document patient interactions ensuring clinical charts within EMR system (NextGen) are kept up to date. Including patient intake, treatment plans, and other relevant information Adheres to HIPPA and patient confidentiality at all times If unlicensed, shall work toward completion of Colorado mental health license within 3 years of initially holding position. If full telehealth provider, shall obtain Telehealth Counseling Certification within 3 months of initially holding position. If providing partial or full telehealth services, shall comply with all Telework Criteria and have an approved Telework Request on file.
