We are seeking a skilled and dedicated Pharmacogenomics Pharmacist to enhance our healthcare team. This position focuses on leveraging genetic information to tailor medication therapies for patients, ensuring personalized and effective treatment plans. The successful candidate will have a strong background in pharmacogenomics or a keen interest in the field, coupled with a commitment to patient care and excellent communication skills. Key Responsibilities: Conduct comprehensive pharmacogenomic assessments to identify how genetic variations affect individual responses to medications. Develop personalized medication therapy plans based on genetic information to improve efficacy and reduce adverse effects. Provide patient counseling and education on pharmacogenomic testing, results interpretation, and implications for medication therapy. Collaborate with healthcare providers to optimize medication therapy and enhance patient outcomes using pharmacogenomic data. Stay current with advancements in pharmacogenomics, including new testing methods and emerging clinical applications. Utilize electronic health records (EHR) and pharmacogenomic databases to document interventions and track patient outcomes. Participate in the development and implementation of clinical protocols and guidelines for pharmacogenomic testing and therapy. Assist with the integration of pharmacogenomic testing into routine clinical practice. Engage in quality assurance programs and continuous improvement initiatives to enhance pharmacogenomic services. Educate healthcare providers and staff on the principles and benefits of pharmacogenomics. Preferred Qualifications: Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program. Experience in pharmacogenomics or personalized medicine. State pharmacy license in good standing. Proficiency in utilizing pharmacogenomic testing platforms and tools. Strong analytical and problem-solving skills. Excellent communication and collaboration abilities. Certification in pharmacogenomics (e.g., Board Certified Pharmacogenomics Specialist) preferred. Familiarity with electronic health record (EHR) systems. Dedication to ongoing professional development and continuous improvement.
