Specializing in delivering top-tier, individualized home care solutions, we prioritize aiding individuals in preserving their independence and enhancing their quality of life.Our consistent dedication to excellence has earned us the esteemed title of "Employer of Choice" by our caregiving team for the past four years, and "Provider of Choice" by our clients for six of the past seven years.Job Description:We are seeking a dedicated and compassionate Part-Time LPNs to conduct comprehensive assessments of clients' health status and develop individualized care plans and train our team of professional caregivers, collaborate with other healthcare professionals, to ensure the delivery of high-quality and elevated level of care for our clients and a learning opportunity for our caregivers.Responsibilities:Client Assessment and Care Planning:Conduct comprehensive assessments of clients' health status and develop individualized care plans.Coordinate with the interdisciplinary team to implement and modify care plans as needed.Client Education and Support:Educate clients and their families about health conditions, treatment plans, and self-care techniques.Provide emotional support and counseling to clients and their families to promote well-being and coping and Reporting:Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and medication administration records.Prepare and submit reports as required by regulatory agencies and company policies.Collaboration and Communication:Collaborate with other healthcare professionals, including physicians, therapists, and caregivers, to coordinate care and ensure continuity of municate effectively with clients, family members, and team members to promote collaboration and achieve positive outcomes.Quality Assurance:Implement and maintain quality assurance processes to ensure compliance with regulatory standards and company policies.Monitor and evaluate administrative procedures to identify areas for improvement and implement solutions.Qualifications:Valid LPN license in the state of ConnecticutMinimum of 5 years of nursing experience, preferably in home care or a related setting.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal abilities - verbal and written.Ability to work independently and as part of a multidisciplinary mitment to providing compassionate care and promoting client well-being.Prior management experience, specifically coaching and motivating employeesExperience with geriatric populationExperience with intakes and initial assessments of prospective clientsExperience and comfortable with presenting in a large group settingKnowledge of In-Home care rules and regulationsBenefits:Competitive pay based on experience.Opportunities for professional development and growth within the company.Rewarding work environment focused on making a difference in the lives of clients and their families.Job Types: Part-timeCompensation: $30 - $35 / HrSchedule: Part-time position, 2-20 hours per week, flexible schedule, including weekends and evenings as neededWork Location: In-PersonJoin our team as a Part-Time LPN and take your career to the next level.Note: This job description is intended to provide a general overview of the position.
