Job Position: Registered Nurse Case Manager (RN) Bilingual Location: Chicago, IL 60608 Pay Rate: $39.00/hr - $44.23/hr based on experience Shift: M-F, 8 AM to 5:00 PM (Flexible) Description: Bilingual (English and Spanish, or Cantonese, or Mandarin) Role Requires 20% Remote and 80% in office JOB RESPONSIBILITIES Identifies, tracks and manages care for the highest risk patients Conducts initial care coordination screening eligibility and performs comprehensive risk assessment Works directly with patients/family members and providers to develop and implement an individualized and integrated patient care plan for each of the highest risk patients. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans Adapts practice to meet individual patient circumstances with regard to health literacy, deficits as well as cultural and linguistic differences, ensuring that effective communication is achieved, particularly where there may be barriers to understanding Utilizes motivational interviewing to gain cooperation from patients and families around the care management plan Work alongside medical home staff at clinic site Education/Experience: RN's Graduate from an Accredited School of Nursing. Bachelor's degree in Nursing required. 3-5 years of experience with 2-3 years years clinical nursing experience in disease management or care management in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred. Bilingual (required): Ability to read, write, and speak proficiently using the both English and one of the following languages: Spanish, Cantonese, or Mandarin Knowledge of healthcare and managed care preferred. Prior experience working in a remote role is preferred. Prior case management experience preferred. Licenses/Certifications: Current state's RN license (IL) both active and in good standing. Required Skills: Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word.
