Employment Type: Full time Shift: Day Shift Description: Are you looking for a Full time RN Admissions Facilitator position with-in the St. Peters Network? Here is your opportunity. We are looking for an RN to help coordinate a smooth transition from our hospitals to our skilled nursing facilities. Position Summary : Working in the Acute Care hospitals, the SNF Admission RN has the responsibility and accountability to collaborate with Central Intake (CI), and SPHP Care Coordination staff to ensure a smooth transition of patients from acute care to the SPHP skilled nursing facilities. The position will work with CI and Care Coordination to ensure that patients admitted to SPHP skilled nursing facilities are clinically appropriate and that timely, complete and useable information follows the patient from acute to post-acute care. Schedule: 10-6pm (Monday through Friday) Location: St. Peters Hospital - Albany New York Education Requirements : Graduate of a Registered nursing school Current New York State registration as a Registered Professional Nurse Experience Requirements : 1-year experience in sub-acute or long-term care preferred Working knowledge of Point Click Care (PCC) and EPIC EHRs Strong clinical assessment and care planning skills Comprehensive understanding of the SNF admission process and associated regulations Demonstrated interpersonal skills to effectively communicate High level prioritization skills. Ability to work in an autonomous organized, creative and collaborative manner Knowledge of interdisciplinary team approach to care management and nursing home regulations preferred Pay range: $33.10-45.75/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
