Hand In Hand Development, Inc., CMA is a NYS DOH approved Early Intervention Agency that provides evaluations, service coordination, and therapeutic services for children birth to 3 in all 5 boroughs, upstate and Long Island. Hand In Hand is also an approved Care Management Agency (CMA) under the lead health home of the Collaborative for Children and Families (CCF), and provides comprehensive care management, health promotion, transitional care, and family support for children and young adults from birth to age 21. Hand In Hand Development is the largest CMA in New York and is recognized as a leader and innovator in the provision of care management services. We are seeking NYS Licensed Speech Language Pathologists to provide therapeutic services in home settings. Our In-Home Services are provided throughout the 5 boroughs of NYC. We utilize a team centered approach fostering collaboration across all disciplines. Salary Range: Fee for Service Location: Throughout 5 Boros $65 for 30 min session, $85 for 60 min session without DOH Letter $70 for 30 min session, $90 for 60 min session with DOH Letter Essential Duties and Responsibilities: Work 1:1 with children diagnosed with developmental delays Collaborate with families to create programs and work towards goals listed in the child's IFSP Monitor & assess program effectiveness Document & communicate treatment results & individuals progress to relevant parties Work effectively with our team of multi-disciplinary service providers Education and/or Work Experience Requirements: Master's in Speech Language Pathology (Required) NYS Speech Language Pathologist License (Required) CPR Certification (Preferred) Previous experience with children who have disabilities and families Ability to build rapport with children & families Passion to help & motivate others Excellent written & verbal communication skills Bilingual a plus Equal Opportunity Employer Physical Requirements: Ability to travel to numerous locations to meet with clients, attend meetings and visit healthcare facilities.
