Job Description: As a POC Nurse Coordinator, your primary responsibility will be to ensure adherence to quality standards and regulatory requirements. Your duties will include, but are not limited to, the following: 1. Quality Assurance Review: • Review and approve the daily Quality Assurance (QA) reports to ensure compliance with established standards and protocols. • Identify any discrepancies or issues and collaborate with relevant stakeholders for resolution. 2. Plan of Care (POC): • Ensure that PDN patients are recertified every 60 days in accordance with company policies and regulatory guidelines. • Conduct recertification and update Plan of Care (POC) for PCA patients every 60 days, adhering strictly to company protocols. • Ensure that PPEC patients are recertified every 180 days in line with company and regulatory requirements. • Coordinates the completion of Plan of Care in an accurate and timely manner, in accordance with policies and procedures, State and Federal requirements. 3. Start of Care (SOC): • Ensure that Start of Care (SOC) assessments are performed when the patient has obtained insurance authorization, in line with regulatory requirements. 4. Document Administration: • Complete the Plan of Care (POC) documentation accurately and promptly within the Axxess system or Acentra/EQ for authorization. • Obtain signatures from family members on the company's contract document during patient visits. • Effectively communicates with Director of Nursing Clinical Managers, Doctors etc., involved in the care planning process. • Conducts regular reviews of patient records. • Responsible for managing the administrative components of patient care plans. • Perform administrative duties such as completing medical forms, reports, etc., as necessary. • Any other duties that may be assigned. Qualifications: • Must possess a current, unencumbered, active license to practice in the state of Florida. • Must have pediatric experience. • Strong understanding of healthcare regulations and compliance standards. • Proficiency in using healthcare software systems, particularly Axxess. • Proficiency in Microsoft suite • Excellent communication and interpersonal skills. • Detail-oriented with a commitment to accuracy and thoroughness. • Ability to work effectively both independently and collaboratively within a team environment. #ZR
