Summary: ***UP TO $15,000 SIGN ON BONUS*** This position delegates responsibility to the Physical Therapist Assistant and Aides. Activities of this position are patient focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures. Responsibilities: (25%)* Evaluates the patients function by performing formal/informal testing measures to determine the skeletal, neurological, respiratory, and cardiovascular status. (25%)* Develops a goal specific treatment regime based on assessment data and the patients tolerance to carry-out the treatment plan as prescribed. (25%)* Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status. Co-signs verbal orders received by physical therapist assistant as necessary. (25%)* Devises a schedule of treatment, which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff. Certifications and Licensures: * Doctorate Physical Therapy, preferred * APTA membership preferred * Physical Therapist State of Michigan (new grads will be considered awaiting state exam) * Basic Life Support Required Education: Bachelors Degree Physical Therapy Other Information: Educates patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions. Provides supervision for Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides and students per subsidiary and regulatory standards. Recommends adaptive/supportive equipment based on assessment; contacts appropriate representative and/or supplier to place order and assists in securing equipment for patient. Documents the patient/family/caregiver response to treatment and changes in patient condition per subsidiary documentation policies and/or standards and in compliance with regulatory agencies. Completes daily documentation necessary for billing procedures per subsidiary policy and in compliance with regulatory agencies. Provides treatment intervention and demonstrates competence to facilitate or restore optimal functioning specific to therapists patient care area. Educates patient/family/caregiver in home education program using relevant and effective teaching methods. Demonstrates effective oral and written communication with all internal and external customers to maximize quality of patient care including coordination with interdisciplinary team members. Promotes education and development among staff, students, and other health care providers using relevant and effective teaching methods. Promotes process improvement. Assists with program development. Participates in team conferences and interdisciplinary meetings as appropriate. Promote physical therapy within MyMichigan Health and service area, participating in and providing community education. Function as case manager for assigned cases. Demonstrates commitment to MyMichigan Health and specific subsidiary mission as evidenced by customer service, quality of service, and teamwork. Maintains confidentiality of patient and affiliate information in accordance with MidMichigan Health and affiliate policies and procedures. Other duties as assigned. Equivalent Experience and Other Comments (Education): Successful completion of MyMichigan Health and affiliate specific orientation. Successful completion of new hire competency. Interpersonal skills needed for effective verbal and written communication in the English language to interact with people individually and in a group at all levels. Ability to work independently, demonstrating problem solving and organizational skills, time management skills and initiative. Mandatory Occupational Safety and Health Administration (OSHA) is required and must be completed annually according to the Exposure Control Plan. Ability to work where employee may be exposed to communicable diseases. Ability to use personal protective equipment correctly. Ability to perform essential job functions with reasonable accommodation. Ability to demonstrate and teach selected treatment routines to appropriate professional and non-professional individuals. Other Comments: Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MidMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Medium. Must be able to occasionally (0-33% of the workday) lift or carry 21-50 lbs., frequently 34-66% of the workday 20 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls and constantly (67-100% or the workday) 10 lbs. #LI-AD1 #Indeed1HP
