RN - Case Manager PRN

Description: Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patientsatisfaction, cost efficiency, and compliance.Reports to: Director of Case ManagementQualifications: Minimum EducationAssociate's or Bachelor's degree or RN Diploma in Nursing - RequiredRequired SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must beable to work in a stressful environment and take appropriate action.Certifications:BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) - Must be obtainedwithin 90 days, if not current upon hireLicenses:Current Tennessee or Multi-State RN licensure requiredMinimum Work ExperienceMinimum 3 years experience in acute care nuring, case management or related field required.Minimum 1 year experience in case management within an acute care or critical access hospital preferred.
