This teaching will include a discussion of quality analytics for skilled nursing facilities. The Discharge Support Specialist must possess critical thinking skills and the ability to anticipate barriers to discharge. The Discharge Support Specialist must be comfortable collaborating with an interdisciplinary team. There will be on[1]going communication with patients and families, post-acute care facilities, and the interdisciplinary team. The Discharge Specialist must be able to multi-task, manage numerous priorities, and remain productivity while managing interruptions. Tasks Review all patients with recommendation for acute care discharge to destinations other than their home setting. Specific focus on patients with a discharge plan to acute or sub-acute level of care.
