The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs. RESPONSIBILITIES: Conduct assessments of maternal and child health needs, to identify barriers to treatment and encourage compliance with medical care Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care for patients Create plan for care across the continuum, integrating patient/family preferences and values. Serve as a resource to assess, evaluate, recommend legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs. Advocate for resources and removal of barriers Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Collaborates with healthcare providers, community organizations, and families to promote health education, preventive care, and effective health management strategies. Educate families about prenatal care, infant care, safe sleep, and Post Partum Mental Health Maintain accurate records of patient interactions and outcomes in compliance with hospital policies. Provide excellent communication skills with the ability to engage effectively with diverse populations. REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Master's in Social Work required. 2 years of social work experience in a hospital setting with maternal child health, children and families is required. LMSW or LCSW/LCSW-C required. MD Social Work License per level of education. Demonstrates the ability to follow verbal instructions, as well as the ability to communicate effectively both verbally and in writing.
