DescriptionACSW/LCSW JOB SUMMARYProvides evidence based screening and psychosocial assessment and brief intervention treatment as part of the clinic health care team. Provides clinical services, mental health prevention and case management for patients and families within a framework of patient centered care, integrating medical diagnosis, social determinants of health and emotional, mental, and substance use disorders.Acsw (associate Clinical Social Worker) Essential Duties And ResponsibilitiesAccurate assessment of mental health issues of patients referred by Clinic PCPs.Direct clinical work with diverse patient population of all agesScreen patients using evidence based screening tools and develop a person centered treatment plan with the patient.Assess patient motivation to engage in substance use and/or mental health treatment and engage or refer accordinglyProvide short term clinical intervention designed to support patient's ability to meet his/her goalsProvide mental health prevention services for patients who are identified as at risk for mental illness and emotional distressProvide case management services, as neededDocument all assessments, interventions and treatment plans appropriately and according to departmental, clinic and payer guidelines.Contribute to a team approach promoting and supporting integration between Primary Care and Behavioral Health.
