Job Summary: This position is a licensed practical/vocational nurse role designed to provide direct nursing care to patients in a post- acute care setting in a contract capacity. LVN will work closely with other permanent and temporary staff and assist in their duties as needed. Essential Job Duties: Contributes to the assessment of patients by collecting data for analysis by an RN. Performs plan of care interventions, including specimen collection and wound care. Demonstrates proper infection control practice and adheres to Infection Control Policies and Procedures and OSHA standards. Adheres to medication pass protocol and meets regulatory guidelines. Evaluates nursing care provided by continual review of the effectiveness of the nursing care plan. Collaborates with supervising nurse (when present) to make appropriate revisions to achieve the desired outcomes. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Works as an advocate for the physical and emotional well-being of the patient. Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families. Collects and documents patient information as needed (including weights and vital signs). Provides patient and family education in regard to the hospitalization/admission, medications, and/or in preparation for discharge. Upon discharge, provides appropriate discharge instructions to include follow-up and referral needs. Provides accurate and timely hand-off communication. Serves as clinical and administrative resource to staff, residents, and families regarding nursing services and procedures, aging processes, medications, and outside medical services. Supervises CNA's and Caregivers. Proficient with computer systems. Job Requirements Education/Experience: Qualified candidates will have graduated from an approved Licensed Practical Nurse program. Must possess a current, unencumbered license to practice as a LPN in this state. Licensure/Certifications: Current LPN licensed in good standing by their State Board of Nursing. Current BLS or CPR/First Aid; Food Handlers Card. ADA/Physical Demands: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Ability to assist in evacuation of patients in the event of a fire or other disaster Routinely works with potentially infectious specimens and patients. Ability to respond to patients in a timely manner and access all areas of the unit. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when a patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other team member, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
