Job Summary: The School Age Service Coordinator will provide case management and support with family maintenance through support and educational planning. SA Service Coordinator's primary focus is supporting family unity and maximizing educational opportunities.Essential Job Functions:The School Age Service Coordinator will provide case management and referrals as appropriate to social service areas of early childhood education, after school care services and programs, youth and young adult development, financial education, workforce development, community building, and health and wellnessSchedule and facilitate IPP, annual reviews, semi-annual reviews, transition planning conferences, clinical consultations, and ID/Team meetings as requiredProvides resources to participants of age-appropriate activities and events..Support children we serve by maintaining regular contact and doing Individual Program Plan (IPP).Provide information and referral to generic services and resources in the communityInvolve individuals we serve and family in all aspects of person-centered planning and support activitiesProvide resources to help families with advocacy, education, collaborates in crisis, intervention, and other support for families and individuals.Serve as liaison to service providers, care homes, schools, and other community agencies as designatedCoordinates the Person-Centered Planning process with individuals and their familiesDevelops the Individual Program Plan (IPP).Counsels individuals concerning identification and achievement of personal goalsAdvocates with individuals/families to assure provision of needed servicesInitiates and follows up on referrals to service providersIdentifies areas of unmet needsPerforms tasks which relate to fiscal and accountability requirementsCompletes all required caseload documentationDevelops expertise in accessing generic resourcesAssures that all individuals and families are informed verbally and in writing of their right to appeal every time a decision is made to change, alter, or deny a serviceKnows Title 17 and Title 22 licensing and DDS requirementsShares all resources that serve our individuals who reside in various counties.Supports in crisis situations in order to eventually effect stabilityPerforms annual, semi-annual or quarterly visits as to residential facilities or day programs as applicableEvaluates conservatorship requests and submits reports to the appropriate parties. Conducts required home visits and meetings in various settings.Education/Experience: A Bachelor's Degree in Social Work, Rehabilitation Counseling, or a related field from an accredited college, or a Bachelor's Degree in Social Work plus four years of relevant experience, is required. The specific field of study for the Bachelor's Degree and the post-Bachelor's years of experience may be waived by the Executive Director if necessary to meet a special need of the agency, such as providing services to specific cultural or monolingual groups.Special Skills and Knowledge: Knowledge of principles, objectives and techniques of social work; individual and group behavior, human growth, development of family relationships; medical-psycho-socio-economic factors affecting individuals and families; techniques of intervention, referral and follow-up to community resources and facilities and various community organizations, and how to intervene constructively on the client's behalf. Ability to relate in a constructive and enabling manner to a wide variety of individuals and organizations; ability to relate in a positive manner with supervisor, co-workers, and others in the agency and community; establish and maintain rapport with clients and their families; prepare and present oral and written reports; maintain case records in a clear, concise, appropriate and timely manner.Other: Use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law, OR ability to provide for independent transportation.
