Your Role The Care Management team will serve to support the mission of the department, which is to provide support to members in maintaining health and wellness in the outpatient setting. The supervisor of Care Management will report to the Senior Manager of Care Management. In this role you will lead the outreach and engagement program to identify members who would benefit from care management services and ensure that members are referred to the appropriate programs based on their individual needs. You will oversee the coordination of care for members, ensuring all services are delivered seamlessly and effectively. Your Work In this role, you will: Provide leadership and oversight to clinical support staff (CSC) involved in care management. Foster a great place to work communicating clear roles and responsibilities and building successful working relationships across the organization. Ensure compliance with all regulatory and accreditation requirements related to care management services. Supervise, mentor, and develop the outreach and enrollment team members, fostering a collaborative and high-performance work environment. Conduct regular performance evaluations and provide constructive feedback. Oversee the identification and enrollment of eligible members into appropriate care management programs. Utilize an auto-dialer to streamline outreach efforts and engage a targeted population for program participation. Ensure the accuracy and completeness of enrollment documentation and maintain up-to-date records. Organize CSC workload and establish work standards to promote efficiency and productivity. Analyze enrollment and program data to track performance metrics and outcomes. Use data-driven insights to inform decision-making and optimize program effectiveness. Assist the Manager with vendor relationships and other projects as needed. Keeps team focused on specific and measurable performance goals/productivity, and monitors performance against clear standards. Works collaboratively among business units to align and partner with others to achieve performance goals and/or outcomes. Your Knowledge and Experience Requires a minimum of 5 years of prior relevant experience Requires a minimum of 2 years of experience operating in a lead role or equivalent leadership training or manager training Experience working with the Commercial and Medicare population and knowledge of managed care highly desirable Detail-oriented with strong organizational and time management skills. Proficiency in data analysis and interpretation to guide decision-making and process improvements. Pay Range The pay range for this role is: $ 65890.00 to $ 98890.00 for California. Note Please note that this range represents the pay range for this and many other positions at Blue Shield that fall into this pay grade. Blue Shield salaries are based on a variety of factors, including the candidate's experience, location (California, Bay area, or outside California), and current employee salaries for similar roles. #LI-AG3
