We are looking for *Speech Language Pathologist *for *part-time or full-time positions and home care.* *Pediatric Home care cases available all over New York City and Long Island,* inquire to find out if there are cases available in your *preferred zip code!* *- No required minimum number of cases for the home care.* *- Min of 45 mins for the evaluations and 30 mins for the revisits/treatment session* *- Flexible schedule* *- Work at your preferred zip codes.* -*$70-$80 per visit (depending on W2 or 1099) ($88 for cases in the Bronx)* *The following are the available locations for SNF:* *Rate: $53-60/hr. (Depending on W2 or 1099)* * Far Rockaway, *11691* * Brighton Beach, *11235* * Coney Island, *11235* * Brooklyn, *11238, 11235, 11224* * Bronx, *10469, 10468* * Jamaica, *11432* * Yonkers, *10701* *Benefits:* * Opportunity to work as an Independent Contractor (1099) or W2 * Assistance in 1099 set up for full-time employees * Attractive Referral Bonus * Weekly Direct Deposit *Responsibilities of a Speech Language Pathologist:* *Skilled Nursing Facility* The SLP will provide and direct Speech Therapy services to patients to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician's orders. * Conduct screenings of patients at regular intervals to determine need for intervention/treatment. * Evaluates patients to obtain data necessary for treatment planning and implementation. * Interprets and communicates evaluation findings to patients, family, and other team members. * Establishes individual patient schedules in conjunction with other team members. * Develops treatment plans by identifying a problem list, including short- and long-term goals and methods to achieve identified goals. * Coordinates treatment plan with patient, family, and other team members. * Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders. I * Instructs patient, family, and staff in use of augmentative communication devices. performance of the patient in their own environment. * Documents results of patient's assessment, treatment, follow-up, and termination of services. *Home Health Care * We provide our patients with the best rehabilitation services, including Physical Therapy, Occupational Therapy, and Speech Therapy, in the comfort of their own home. * Administers speech and language therapy treatments to patients as per physician's orders using modalities and techniques appropriate for the individual patient. * Makes an initial and periodic evaluation of the patient's speech and language and rehabilitation needs. Sets long and short-term goals in measurable terms and adjusts treatment plans accordingly. * Consults with the Rehabilitation Therapies Coordinator and nursing staff to make joint decisions regarding continuation of, or dismissal from care when the need for speech therapist treatment is no longer necessary. * Assist with the orientation and/or guidance of new staff members professional and non-professional, nursing students, volunteers, and special visitors. *Qualifications of a Speech Language Pathologist:* * Master's Degree in Speech-Language Pathology * Licensed in the State of New York Grab this opportunity by securing this position today!
