Plan of Care Following the initial plan of care completed by RN, reviews and updates care plans per policy. General Patient Care Implements, documents and provides care and education based on the plan of care.  Documentation Evaluates and documents patient care outcomes. Teamwork/Professionalism Demonstrates professionalism, integrity and accountability in the LPN role. Patient Safety Complies with LifeBridge patient safety mission, policies and procedures. Certifications, Licenses, Registrations: American Heart Association CPR Certification License for Practical Nursing Wound Care Cert within 2yr of hire.
