Clinical Pharmacist Lead

Our team at Alton Memorial Hospital is expanding and is looking for a patient - centered pharmacist to join our dynamic team!  In this role, you will make a direct impact on patient care by rounding with a multidisciplinary team, reviewing patient medications, providing patient education/counseling, and more.    Monday - Friday  8 AM - 5PM Overview Since 1937, Alton Memorial Hospital has cared for residents in Alton, Illinois, and the surrounding communities in a five-county area. Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more.   Preferred Qualifications Role Purpose Assess, monitor, initiate, and modify medication use according to patient condition and age specific criteria. Serves as a liaison between the pharmacy department and other health care practitioners in matters related to specialized area of pharmacy practice. Supervises and directs supportive personnel. Counsels and educates to promote optimal drug therapy. Directs and assists in the procurement, storage, distribution and disposal of medications.   Responsibilities Evaluates medication orders for appropriateness with regard to patient condition, organ function and concomitant therapies to promote optimal patient outcomes. Documents all activities and communication as appropriate. Directs and manages workflow in focused area of pharmacy practice. Activities include oversight of pharmacists, technicians, interns and volunteers ensuring the accuracy of their work in accordance with departmental procedures and Federal and State regulations. Participates in the delivery of educational programs within and outside of the pharmacy department. Serves on committees at the health service organization. Serves as a lead on the development, review, and implementation of policies and procedures. Provides advanced level support to the professional, operational, and patient care aspects of the pharmacy department. Identifies and implements improvements to the medication use process.
