Job Summary The Home Health RN Case Manager provides one-on-one care and skilled services to patients in their own home, contributing to the individuals overall quality of life. They provide in-home evaluation, direction, and application of skilled nursing services to complement the patients overall Plan of Care. Key Responsibilities: Provide in-home evaluation and assessment of patient needs. Develop and implement a comprehensive Plan of Care for each patient. Collaborate with healthcare teams to ensure seamless care coordination. Provide education and resources to patients and caregivers to empower them to manage their conditions. Requirements: Education: Graduate of an accredited nursing program. Experience: Preferred: Minimum one (1) year experience as a professional nurse within the last three (3) years. Skills: Ability to provide reliable transportation as required by the nature of the position. Licensure/Certification/Registration: Required: RN License active in the state of Missouri.
