Position SummaryLizzy Care is seeking an accomplished Head of Clinical Program Development and Operations to lead and expand our medical- and clinical-operations, design and implement novel patient-care programs (e.g. dementia, geriatric, palliative and in-home care), and advance our B2B partnerships with healthcare and community-based organizations nationwide. As employee #6 of our growing startup, this individual will partner closely with the Chief Executive / Chief Medical Officer of Lizzy Care, as well as with product and sales teams. S/he will play a pivotal role in driving high-quality care, building a multi-state housecalls provider organization, institute comprehensive clinical workflow and monitoring systems, and will work with the senior leadership team to capture value and drive business outcomes. Duties/Responsibilities:Program Development:Design and implement clinical care management application specifications, including:Standardized, evidence based assessment toolsClinical team-based workflows to drive interdisciplinary careComprehensive care-plan design and implementationPartner with product and tech teams to ensure documentation systems are in compliance with best evidence and CMMI GUIDE Model requirements for dementia care managementDrive systems for discrete data collection to track patient specific outcomesCollaborate on network development for clinical support servicesClinical Team Management:Partner with medical leadership to establish and oversee a multistate housecalls medical practiceRecruit and train members of the interdisciplinary team (e.g., CM/SW/CHW)Lead ongoing case review and IDTMonitor adherence to evidence-based care protocols and pathwaysCoordinate patient and family education resources and deliveryLead patient safety, compliance and risk management programsClient Success:Develop strategic partnerships with healthcare providers and other key stakeholders to enhance service offerings.Collaborate with internal teams to ensure alignment between partnership goals and joint venture objectives.Manage agreements with JV partners, ensuring mutual benefit and compliance with health system policies.Monitor and assess partnership outcomes, providing regular reports and recommendations for improvement.Identify collaboration opportunities to improve patient care, streamline services, and promote growth.QUALIFICATIONSAdvanced clinical degree (ie: RN/NP/MSW/DPT)Minimum 5 years clinical experience post licensureMinimum 2 years experience as a supervisor of clinical teamsDemonstrated experience with clinical principles of geriatric, palliative, care management and in-home careDemonstrated experience with tech-enabled clinical program developmentDemonstrated experience with risk-based, value-based care programs (ie: ACO, bundles, IAH, CMMI innovation programs)Prior startup experience preferredCOMPETENCIES (skills, abilities, knowledge)Collaboration and team functioning - able to work collaboratively as part of a multidisciplinary team, able to proactively collaborate with other supervisors and program directors to ensure the best possible training for supervises and service delivery for clientsAble to work effectively in an organizational setting and understand how to utilize the chain of command to resolve problems and bring program development ideas forwardEfficiency and time management skillsExcellent interpersonal, communication and writing skillsDETAILSReports to: CEO (Dr. Marc)W2 full-time roleDuties: 75/25% Administrative/ClinicalLocation: Hybrid role, 25% in person with leadership team.
