Case Manager - Bronx Scatter Site

A nonprofit services organization in New York City is looking to add a new Case Manager to their staff in the Bronx. In this role, the Case Manager will be responsible for providing case management services to formerly homeless individuals, some with multiple disabilities, in order to help the individual transition and adjust to permanent housing.About the **Ideal Start ** ASAP***** Monday to Friday***** 9am to 5pm** 13 to 16 patientsOversee case coordination for a caseload of 15, including home visits in clients apartments; provide escort to appointments, as neededConduct an initial and ongoing assessment of clients , including completion of biannual assessments and psychosocial assessmentsProvide crisis intervention, when neededConnect clients to an array of services as appropriate Maintain record keeping in compliance with agency standards as well as those of City and State agenciesSupport group facilitation, including planning and participating in events, such as holiday parties, annual BBQ, etc.Perform other related duties as assigned2 years of experience working with people with co-occurring disorders Associates and/or Bachelors DegreeProficiency with Microsoft Office suite and databasesKnowledge of issues of the Homeless populationKnowledge of Social Services systemsMicrosoft Office Suite proficientSolid assessment and documentation skillsGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired **Bilingual (English and Spanish)Training in Harm Reduction, Housing First, Motivational Interviewing, and Crisis Intervention
