A leading healthcare organization in New York City is seeking a dedicated Stroke and Sepsis Coordinator to oversee the coordination, maintenance, and growth of its Primary Stroke Certification Program. This role focuses on quality improvement, staff education, and community outreach to enhance stroke and sepsis care and outcomes.**The Stroke and Sepsis Coordinator will be responsible *Coordinating, maintaining, and expanding the Primary Stroke Certification Program in collaboration with key stakeholdersConducting monthly Stroke and Sepsis Program Committee meetings to evaluate outcomes, policies, and protocol complianceCollecting, analyzing, and maintaining stroke and sepsis data to improve patient care and performance metricsOverseeing data submission and reporting to regulatory bodies such as CMS, The Joint Commission, DOH/Coverdell, and GWTGsProviding ongoing staff education on stroke and sepsis management, prevention, and best practices across departmentsDeveloping patient education materials and collaborating with community organizations to promote stroke awareness and prevention programsCoordinating with ED and EMS providers to ensure access to and compliance with emergency stroke and sepsis treatment protocolsSupporting patient experience initiatives by analyzing stroke-specific satisfaction surveys and enhancing care deliveryServing as a resource and support for stroke patients and their families, offering education and guidance*Bachelors degree in Nursing or related fieldCurrent NYS RN licensure; advanced certifications in stroke care (e.g., SCRN) or sepsis management3-5 years in stroke care, sepsis management, or related clinical areasStrong knowledge of stroke and sepsis protocols and best practicesProficiency in data analysis and performance improvement metricsExperience in staff training and education program developmentExcellent communication and interpersonal skills for collaboration across departmentsDemonstrated ability to manage multiple priorities effectivelyDesired **Masters degreeFamiliarity with CMS, The Joint Commission, and other regulatory reporting standardsExperience working with EMS providers and community outreach programsProven ability to lead quality improvement initiatives
