Licensed Master Social Worker

A healthcare organization in New York City is currently seeking a Licensed Master Social Worker (LMSW) to join their growing Inpatient Medicine Department. In this role, the Social Worker (LMSW) will be responsible for identifying psychosocial issues and concerns of patients and their families as well as providing appropriate clinical services to meet their needs.About the **Weekend shift only 8am to 6pm Acute Care Inpatient Medicine Epic EMRsScreen charts for historical and clinical informationConduct psychosocial evaluations through interviews with patients and/or identified surrogatesFormulate appropriate social work treatment plansAssist in plans for institutional or hospital discharge and other post institutional/hospital care; evaluate alternate care arrangements for patients, where neededConfer and consult with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care and to develop treatment plans, evaluate patients' progress, develop discharge and aftercare plans and recommend alternative action or treatmentProvide outreach services, make referrals to other governmental, public and/or community agenciesPrepare patients to accept services, provide direct social work services on high-risk cases and escort patients in connection with community activities and referralsPrepare reports and related paperwork; keep appropriate recordsPerform other duties, as needed1+ year of Social Work experience in a Hospital settingMaster's DegreeNYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)Desired **BilingualWorking knowledge of Epic EMRs
