Leading a dedicated team of care coordinators, your role will involve ensuring that each member receives personalized support and seamless access to essential services. Your leadership will foster collaboration among healthcare providers, improving health outcomes and overall member satisfaction. By utilizing your expertise in care coordination, you will help develop innovative strategies to address the unique challenges our members face.The Care Coordination team is committed to ensuring Medicaid beneficiaries receive the comprehensive dental care they need through effective coordination. They assess each members unique needs and create personalized care plans that connect them with the right dental providers and resources. The primary goal is to advocate for the members by facilitating communication between them and healthcare professionals, helping them navigate the complexities of the healthcare system. By working with various stakeholders, they aim to improve health outcomes and enhance the overall client experience. Ultimately, the team is dedicated to making dental care more accessible and effective for those served. As the Bilingual Complex Care Coordination Manager, you will lead a team of up to 10 Complex Care Coordinators and 15 Care Coordinators.ResponsibilitiesProficient in managing complex care coordination processes, ensuring comprehensive support for membersAbility to assess individual needs and develop tailored care plansStrong understanding of dental health issues, Medicaid regulations, and interdisciplinary healthcare deliveryExperience navigating the healthcare system to facilitate access to servicesSkills in analyzing data related to care coordination outcomes, identifying trends, and recommending improvementsProficiency in using data management tools and reporting softwareKnowledge of Medicaid guidelines and best practices in care coordinationAbility to ensure compliance with state and federal regulationsStrong leadership and team management skills, with the ability to motivate and inspire team membersExperience in developing training programs and fostering a culture of continuous improvementExcellent verbal and written communication skills for effective interaction with members, providers, and team membersAbility to convey complex information clearly and empatheticallyStrong analytical and critical thinking skills for assessing complex situations and developing effective solutionsAbility to manage multiple priorities and adapt to changing circumstancesStrong relationship-building skills to foster trust and collaboration among team members and stakeholdersAbility to advocate for members and effectively address their needsRequirementsBachelor's degree in Dental Hygiene or a related field5+ years of experience in healthcare management, care coordination, or case management, preferably with a focus on dental servicesProven ability to lead high-performing teams in a metric-driven environmentTrack record of successfully meeting key performance metrics and service level agreements on a monthly and/or quarterly basisDemonstrated focus on coaching and developing team members, ideally through a structured coaching and quality programStrong skills in Microsoft Excel, including the ability to create and analyze pivot tables, charts, and basic formulas for data reporting and analysisProficient in Microsoft Office tools, such as Word and PowerPoint, for documentation and presentationsAbility to maintain confidentiality and adhere to HIPAA requirementsExcellent verbal and written communication skills, with the ability to document and present information empathetically and clearlyMust be adaptable, flexible, and able to adjust to changing situations with diverse teamsHighly organized, detail-oriented, and a self-starter with strong analytical skills and the ability to multitaskStrong knowledge of Medicaid policies and procedures related to dental care, preferredCertification in Care Coordination or Case Management (eg, CCM, CMC) is a plusBenefitsOur client provides meaningful and challenging work that fosters innovation in a secure and compliant environment, alongside a competitive compensation package.
