Occupational Therapist, OT ***** Assessment and ***** Perform comprehensive assessments of residents functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Individualized Care ***** Develop tailored intervention plans to improve residents independence in daily activities. Section GG Data ***** Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments. ***** Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications. ***** Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care. ***** Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC). Interdisciplinary ***** Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
