Medical Coordinator Overview: We are seeking a highly organized and detail-oriented LPN/Medical Coordinator to join our team. The LPN/Medical Coordinator will be responsible for managing patient care and ensuring that all medical services are provided in a timely and efficient manner. This role requires excellent communication skills, a strong understanding of medical procedures, and the ability to work in a fast-paced environment. Duties: Prepare consult sheet for each appointment regardless of locale per agency guidelines: Review all consults after appointment immediately upon return to facility (whether medical coordinator accompanied the consumer or not). Submit Literal Text changes, new or discontinued orders to ChemRx by the 15th of each month to ensure accurate medication record on next month's MAR. Meet weekly for Medical Meeting with RN and Manager: Review any outstanding tasks, diagnostic tests, appointments to be completed. Attend Clinical Team Meetings: ICF: Quarterly, Annual and semi-annual as scheduled and/or monthly clinical IRA: Semi-annual, annual and as needed. Organize Consent for treatment process: Coordinate completion of documentation with RN, manager, MD Any additional tasks related to consumer health care or medical system as deemed necessary by RN or Nurse Administrator. Prepare new monthly MAR: Daily review of MAR/ Medication inventory to ensure: Ensure all medication errors are documented on an occurrence report and reported to the RN and manager. May be assigned other tasks and duties Other Functions: Demonstrate respect for all people Demonstrate professional demeanor, attention to punctuality and attendance policies, reliability, flexibility and pleasantness Demonstrate privacy on all medical concerns Professionally interacts with other staff members and with others in the community Demonstrate enthusiasm for learning the knowledge and skills required to perform the job Readily seek and accept feedback to improve performance Apply knowledge and skills gained on the job and maintain required certifications Participate in the work of the organization in a positive way by using problem solving skills Demonstrate respect for the safety of all others Perform additional responsibilities as assigned by the Senior Staff May be assigned other tasks and duties reasonably related to their job responsibilities Experience: Minimum 2-year experience working with developmentally disabled population preferably OPWDD population with in a residential setting . To serve as Medical liaison between physicians, ancillary services and the individual/ IDT team and family; works with consultants, dietary personnel, pharmacist, lab, etc as needed to implement recommended changes, and will be responsible for documentation of their visits. Physical Demands: Regularly required to speak clearly and hear the spoken word well. Regularly required to utilize near vision ability to read data and documents, including spreadsheets and reports, in printed form and on computer screens. Regularly required to provide physical care for clients and individuals, which may include, but is not limited to bathing, feeding, changing clothes or diapers and pushing wheelchairs. Ability to physically restrain challenging behaviors that include aggression, self-injurious, and destructive behaviors. Ability to apply proper lifting techniques Work Environment: Noise level is consistent with levels usually present in an office, education, group home, residential, rehabilitation, industrial, workshop or health related environment. Hazards present are consistent with those common to an office, education, rehabilitation, industrial, workshop or health related environment. Working with individuals whose age ranges from childhood to adulthood Requirements: - Valid NY LPN License required. - Ability to work in a fast-paced environment and prioritize tasks effectively. - Excellent communication and organizational skills. - Certification in medical coding or a related field is a plus. - Experience in case management and health coaching is highly desirable.
