Opportunities for Advancement STATEMENT OF THE JOB * Under the direction of the Certified Community Behavioral Health Clinic (CCBHC) Medical Director, the Nurse Practitioner CCBHC will provide primary care screening and monitoring services to CCBHC patients. The Nurse Practitioner CCBHC will have regular interactions with key CCBHC Demonstration program staff to ensure coordinated, whole-person care on behalf of CCNS' CCBHC clients. The Nurse Practitioner CCBHC will refer individuals out for any additional primary care or specialty services needed. • The Nurse Practitioner CCBHC will provide the allowable CCBHC primary care screening and monitoring services, including: o Health Physicals (New or Established Patients) o OMH Health Monitoring (15 min, 30 min, 45 min, 60 min) o OMH Health Monitoring Group (30 min or 60 min) o OMH or OASAS Smoking Cessation Treatment for an Individual (3-10 min or > 10 min) o OMH Smoking Cessation Group (>10 min) • Conform to screening recommendations A and B of the US Preventive Services Task Force Recommendations. • Work closely with the CCBHC Nurses and other staff to ensure that all CCBHC clients are provided screenings related to the following conditions and health risk factors: o HIV and Viral Hepatitis screening o Basic vitals, medical history, and current physical health symptoms o Body Mass Index (BMI), BMI percentile (for children and youth) and Nutrition/Exercise levels o Chronic diseases screening • Analyze lab results and the results of other health-related assessments, in order to monitor the health conditions of CCNS' CCBHC clients. • Support the identification and ongoing monitoring of individuals served by the CCBHC who are living with chronic health conditions who need additional primary care or specialty support services. • When needed (e.g., for individuals with complex co-occurring behavioral health and physical health needs), assist with coordination with an individual's external PCP (not associated with CCNS), in order to assure a coordinated Care Plan for clients served. • Assist CCNS to develop and refine policies related to primary care screening and monitoring services. • Provide and/or oversee supervision of staff and graduate students. • Perform other duties as directed. *QUALIFICATIONS • Master's Degree in Nursing or equivalent • New York State Nurse Practitioner License and Current Registration as Family Nurse Practitioner • Current CPR (BLS), First Aid • Apply for and maintain valid NPI Number • Must receive (Medication Administration Training) & become MAT certified within first 6 months of employment • Federal DEA and State Controlled Substance Registration • Must be Board Certified- and either hold the American Nurses Credentialing Center (ANCC) FNP Certification or the American Academy of Nurse Practitioners Certification Board (AANPCB) FNP Certification. • 5 years' experience providing primary care services to individuals living with mental health conditions and/or substance use disorders. • Computer literate with proficiency in statistical software and using electronic health records.
