This role will provide evaluation, treatment, and patient-centered care to individuals recovering from injury, surgery, or chronic conditions.Responsibilities:Evaluate patients and develop personalized treatment plans based on functional goalsAdminister physical therapy interventions in inpatient and/or outpatient settingsEducate patients and caregivers on home programs, safety, and recovery expectationsCollaborate with a multidisciplinary team including physicians, nurses, and occupational/speech therapistsMaintain accurate and timely documentation in accordance with regulations and facility standardsInterested in hearing more?
