The ideal candidate will have at least 1 year of experience as an LMSW and a strong background in discharge planning within a hospital setting. This is a temp-to-perm opportunity, and we are looking for local candidates to ensure immediate availability.Responsibilities:Conduct assessments and provide mental health support to patients.Develop and implement care plans based on patient needs.Manage discharge planning, coordinating services and resources for patients post-discharge.Work collaboratively with healthcare teams to ensure the best patient outcomes.Provide crisis intervention and support as necessary.Qualifications:Valid Licensed Master of Social Work (LMSW) license issued by NYSED.Minimum 1 year of LMSW experience in a hospital or healthcare setting.Experience with discharge planning and coordinating care post-hospitalization.Recent hospital experience is strongly preferred.Strong interpersonal and communication skills.Interested?
