A health services organization in New York City is looking to add a new Case Planner to their team in Brooklyn. In this role, the Case Planner will be responsible for coordinating and monitoring services for clients and is the liaison to all community resources as they relate to discharge and reunification.About the Opportunity:Start Date: ASAPAssignment Length: Contract to Hire Schedule: Monday to FridayHours: 9am to 5pmAssignment Length: Contract to HireSetting: Special Family Foster Care (SFFC) programPatient Population: Ages 0 to 25Patient Caseload: 12 per workerEMR: Connections and NextGenResponsibilities:Create service plans which address safety, behavioral, educational, psychological and permanency needsMonitor safety, well-being and progress against other critical outcomes by facilitating regular casework contacts and by documenting the same in progress notes and other case documentationSchedule and supervises mandated visits between child and natural familyConduct ongoing required interviews in person with child, natural family and foster familyIndependently make all related field visits, to homes, schools, courts, prisons, hospitals and other sites as cases may requireAttend all Family Court hearings on assigned cases; provide testimony regarding case status and Agency positionParticipate actively in Family Team Conferences and other meetings to advance permanency for children and familiesEnsure that the medical and mental health needs of children and the biological family are being monitored appropriatelyPerform other duties, as neededQualifications:Bachelor's DegreeMust have 1 year Case management experience and 1 year Foster Care or Mental Health experienceExperience in Child WelfareExperience with with Mental Health illness affected populations and/or Substance Use Disorder individuals.Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV/ AIDS, Substance Abuse, etc.Extensive knowledge of Case Management principles and practicesMicrosoft office proficientSolid problem solving and time management skillsExcellent communication skills (written and verbal)
