Locations: Brooklyn, NY 11206 Work Setting: In-person (Half-day in clinic, Half-day in home)Schedule: Monday - Friday, 9:00 am - 5:00 pm ETSalary Range: $100-114k/yearAbout the RoleWe are seeking a dedicated Physical Therapist to join our PACE (Program of All-Inclusive Care for the Elderly) team. This role involves planning and administering therapy programs for medically referred patients to restore function, relieve pain, and prevent disability.Key ResponsibilitiesConduct assessments, progress reviews, treatment plans, and discharge summaries.Perform participant evaluations and treatments in clinics, homes, or nursing facilities.Maintain a safe environment and adhere to safety protocols.Collaborate with interdisciplinary teams to develop and implement participant care plans.Document participant information, care plans, and statistical data accurately.Consult with physiatrists and other professionals as needed.Provide clinical instruction to students and volunteers.Facilitate discharge planning, including equipment orders.Participate in in-service training and performance improvement initiatives.QualificationsEducation:Graduate of an APTA-accredited Physical Therapy program (or equivalent).Experience:Minimum of one (1) year as a Physical Therapist.Experience with geriatric or frail populations preferred.Licensure:Licensed Physical Therapist in New York State.Additional Requirements:Medically cleared for communicable diseases with up-to-date immunizations.Strong interpersonal skills to establish rapport with patients and staff.Ability to travel to participant locations.Familiarity with PACE regulations and electronic medical records (Care Compass preferred).Bilingual proficiency preferred based on regional demographics.Why Join Us?Opportunity to make a meaningful impact on the lives of elderly participants.Work within a supportive, interdisciplinary team.Competitive salary and benefits package.
