Medication Safety and Internal Medicine Critical CareTransplantEmergencyInfectious DiseaseJOB SUMMARYThis position employs pharmacists with extensive specialized training/experience as per American Society of Healthcare Pharmacists standards. Specialist is responsible for providing advanced clinical pharmacy services to patients of all ages in the area of specialty. This position is consulted by physicians, pharmacists, nurses and other disciplines to provide recommendations on optimal use of drugs in their area of specialty.PRIMARY DUTIES AND RESPONSIBILITIESPerform day to day clinical pharmacist duties:Conduct comprehensive clinical review of all admitted patients to identify medication related reasons for admission (omission, commission, contraindication, dosing errors etc.)Conduct comprehensive daily safety event review to identify and address drug related harm (code blues, rapid responses, falls, deterioration in medical condition etc.)Conduct comprehensive clinical review of all discharging patients to correct medication related discrepancies.Attend daily interdisciplinary rounds with medical teams on assigned floors to make recommendations for drug therapy.Conduct comprehensive review of medication profiles to optimize treatment regimens considering patient's overall condition (diagnosis, age, co-morbidities etc.)Responsible to identify opportunities for lower the cost of total treatment by recommending discontinuation of unnecessary drug orders, assist in choosing cost-effect but equally efficacious and safe drug therapy, switching patients from expensive and high-risk IV products to oral treatment options.Rotate in all areas of pharmacy operations as assigned.Conduct daily rounds with advance medical teams to provide care for high acuity patients (complicated infections, critically ill patients, transplant, oncology etc.)Act as pharmacist specialist consultant to prescribe medication treatment for high-risk patients in the outpatient clinics under a collaborative practice agreement with the medical institute.Serve as pharmacist specialist and consultant in their expertise to optimize medication therapy for a specific diagnosis or conditions (for example, infectious disease, transplant, critical care, oncology, nephrology, neurology, transitions of care etc.)Assigned to their respective medical institute to guide optimal use of drug therapies for all conditions treated by the medical institute.Assist medical institute and pharmacy clinical leadership in choosing most efficacious, safe, and cost-effective therapeutics for addition to hospital formulary.Conduct Medication Use Evaluation for evidence based safe, effective, and cost-effective drug use at CCAD.Assist in developing clinical training programs for interns, residents, clinical pharmacists, physicians, and nurses.Conduct routine training for physicians, pharmacists, nurses, residents, and others on optimal and safe use of drugs.Assist medical institutes and pharmacy leadership in developing drug-use protocols to standardize medication treatments based on most current evidence in the literature.Responsible for safe drug use in their area of expertiseResponsible to lead the service area in their specialty and own clinical, safety, financial and other goals assigned by leadership.Responsible for care coordination for medication related issues and treatment approach for patients in the service areaLead and foster research activities in the organization across areas of specialty to advance care at CCAD and UAE.EXPERIENCE REQUIREMENTSACPE accredited Pharm.D. With ASHP Accredited Residency with PGY2 in the area of specialty ORPGY1 (North America) at a minimum with 2 years of clinical experience in specialty area comparable to PGY2 residency training and successfully completion of credentialing and competency assessments ORACPE accredited Pharm. D. with 4 years of experience in North America as hospital clinical pharmacist practicing in a specialty area and fulfilling the requirements of duties and responsibilities listed above in this JD. ORPGY1/PGY2 (CCAD) and successful completion of clinical training and competencies of the department to fulfill the requirements of duties and responsibilities listed above in this JD. ORPharm.D. with 4 years of CCAD Clinical Pharmacist II experience and successful completion of clinical training and competencies of the department to fulfill the requirements of duties and responsibilities listed above in this JD.Current national license is requiredEDUCATION REQUIREMENTSDoctor of Pharmacy Degree (Pharm.D.) from an ACPE accredited program (Accreditation Council for Pharmacy Education)Completed at least one (1) year of an ASHP accredited Residency (PGY-1)the ASHP accredited residency is preferredBoard Certification (e.g., BCPS) is preferredJob Specific Skills and AbilitiesAbility to develop ideas and solutions to problems related to both patient care and researchProficient in the use of PCs, spreadsheets and other pharmaceutical applicationsStrong oral and written communication skills`Ability to speak and write in English is requiredAbility to speak and write in Arabic is preferred
