Job Title: Pediatric Medical/Surgical Inpatient Social WorkerJob DetailsPosting Date: December 16th 2024Reference ID: 236917Location: Baltimore, MDWork Location: On-siteDiscipline: Social WorkSetting: Pediatric Medical/Surgical Inpatient HospitalJob Type: Full-Time; Direct Hire; PermanentSalary Expectations/Pay Rate or Range: $70,000 - $85,000 (based on experience and licensing)Schedule/Shift: Monday - Friday, 8:00 AM - 5:00 PMOpportunities: Two positions available - one assigned to a specific pediatric hospital floor, and one floating between floors.Job DescriptionResponsibilities/Job Duties:Oversee medical and surgical patients to ensure timely and effective discharge planning.Support patients in identifying and utilizing community resources for post-discharge needs.Conduct hospital room visits and, in specific cases, virtual sessions for recently discharged patients.Manage a caseload of up to 25-30 patients at a time, with oversight for up to 100 beds.Collaborate during interdisciplinary rounds with providers, utilization review, case managers, and other social workers.Address social needs and advocate for patients in a hospital setting.Minimum Education and Experience Qualification Requirements:Required Qualifications:Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).Open to new graduates with internship experience in a hospital setting.Familiarity with local community resources in Baltimore.Preferred Qualifications:Prior experience in a healthcare or hospital setting.Pediatric social work experience in Baltimore.Experience managing high patient volumes in a fast-paced environment.
