With a growing senior population, we understand the importance of presence throughout the geriatric continuum promoting rehabilitation, prevention, and wellness to facilitate positive outcomes and fiscal success.Physical Therapist are responsible for providing quality direct patient care including: screening, evaluation, development of initial & ongoing treatment plans, discharge plan and case management. Physical Therapist is responsible for the clinical supervision of Physical Therapy Assistant (PTA) and compliance of all professional standards.DESCRIPTIONBuild a strong Physical Therapy programOptimize the functionality and skills of patientsResponsible for the evaluation and treatment of residents after a written order received from physicianScreening of facility residentsCompletes all necessary documentationAttend all required meetings and in-services, including but not limited to family meetings, care plans, staffing, utilization reviewIn conjunction with Director of Rehabilitation, develop programs appropriate to the needs of the facility residents through continuum of careCompliant with all infection control procedures and environmental protocol while following OBRA Resident's Rights GuidelinesProvide consultation and clinical supervision to PTAsMaintain valid state license and awareness of issues related to the profession of PT and healthcare environmentResponsible for appropriate scheduling of residentsFacilitates and completes family and facility staff in-services and educationAssist the facility in meeting standards necessary to maintain state and federal certificationComplete all other tasks that are deemed appropriate for the role and assigned by the manager/supervisorREQUIRED SKILLSExcellent communication skills both written and verbalAbility to maintain professionalism in all situationsStrong time management and ability to manage caseloadAbility to clinically supervise Physical Therapy Assistants according to state regulationsPhysical Demands: Required to perform a variety of duties relative to the job, some of which require a moderate to heavy category of physical demandPush/Pull up to 300lbs, Standing, Sitting, Sustained Squatting, Repetitive Stooping, Trunk Twisting, Sustained Kneeling, Sustained & Repetitive Forward Reaching, Stair Climbing, Walking, Grasping, Fine Motor, LiftingREQUIRED EXPERIENCE AND QUALIFICATIONSMaster of Science Degree or greater in Physical TherapyState licensureDESIRED SKILLS AND EXPERIENCESenior Living experience preferredPDPM knowledgeExperience with therapy software/EMRMedicaid case mix knowledge
