POSITION OVERVIEW:Case Manager / Recovery Coach (Full-Time)Are you passionate about helping people change and improve their lives - with a focus on substance use and mental health? Join us!Williamsburg House is a best-in-class sober transitional living program, serving individuals who are working towards maintaining sobriety and recovery from substance use. As a member of the Williamsburg House Recovery Team, you will provide compassionate, personalized, 1-on-1 client support through guidance, structure, accountability, and achievement of measurable goals.The Case Manager / Recovery Coach removes personal and environmental obstacles to recovery, forges links to the recovery community, and serves as a guide and mentor in the management of personal recovery. This vital role participates as part of a highly dynamic and interdisciplinary team collaborating to facilitate the achievement of the recovery and wellness goals of our residents.ESSENTIAL DUTIES AND RESPONSIBILITIES:Assess and develop individualized recovery and wellness plans that facilitate a smooth transition back to work, home or school while carrying out the clinical team's recommended continuing care planCoordinate care with all clinical treatment providers and relevant family membersAssist in locating personal and recovery-based resources (ie. AA and NA meetings, sponsors, community, wellness and skills development, employment, etc.)Motivate residents and continually monitor and evaluate each resident's progression through their case plan, and revise the plan as neededMonitor various testing reports (ie: toxicology screening) and adherence to schedules for residents and document resultsCommunicate progress to families, treatment teams and managementProvide 1-on-1 coaching and mentoring as necessary to residents as well as other case managers and staffPrepare case management related documentation and reports including but not limited to: attendance, progress, incidents, outcomes, etc.Work as part of the shift team, carrying out program & residential dutiesActively participate in or facilitate resident- and program-related meetings and activities, and foster resident engagement in the house communityAttend program-related community, training and committee meetings as assignedAdhere to strict boundaries and professional codes of ethics in the care of othersConduct crisis intervention as neededQUALIFICATIONS:Education: Bachelors degree in Psychology, Social Work or related field; OR an equivalent combination of related education/work/lived experienceCredentials - one or more of the following is preferredCASAC - Credentialed Alcohol and Substance Abuse CounselorCARC - Certified Addiction Recovery CoachCRPA - Certified Recovery Peer Advocateor similarExperience:At least 2 years working in an addiction treatment or recovery setting is required1-2 years continuous experience working in substance use as a case manager or recovery coachAt least 2 years experience navigating various recovery support programs (12-step, SMART, Refuge etc.)Additional QualificationsDirect experience working with substance use and co-occurring mental health issues, LGBTQIA+, eating disorders, SLA, crisis intervention, and trauma informed careExperience collaborating directly with client families and providersDetailed knowledge and experience with the Stages of Recovery and the recovery treatment continuum - detox, inpatient, intensive outpatient, sober living, aftercareUnderstanding of Medication Assisted Treatment (MAT)Skills:People person, excellent time management skills; self-motivated to achieve objectives and goals, able to manage stress, effective at conflict resolution, flexible; team player; open to feedback; creative problem solver; practices good self care, excellent organizational skillsStrong ability to communicate effectively to a wide audience, including parents, clients and treatment professionalsPassion for helping individuals in recoveryWillingness to perform toxicology screening, CPR, emergency first aid, Naloxone, crisis prevention and de-escalationComputer skills: proficient in Microsoft Office (specifically Word & Excel), EMRs, communication & collaboration tools (ie.
