 Job requires 2/3 days remote Telehealth AND 2/3 days in home assessments - MUST LIVE IN LOS ANGELES CALIFORNIA Join a physician-led and clinician-driven program focused on providing compassionate care for patients with complex, chronic needs. As an Advanced Practice Clinician, you will work with an interdisciplinary team to conduct in-home and virtual assessments. Can be an NP or a PA. Schedule: Monday - Friday, 8 am - 5 pm Responsibilities:Perform in-home and telehealth assessments, including physical exams, cognitive and safety.Facilitate end-of-life care discussions when appropriate.Collaborate with primary care providers on care gaps and communicate assessment findings.Provide education to patients and families with a focus on follow-up and community resources.Qualifications:Master's degree from an accredited NP or PA program.Active California NP/PA license, BLS certification, and valid California driver's license.Open to new grads with a desire to learn.
