Brief DescriptionFunctional TitleLicensed Clinical Social WorkerGeneral Summary: Under the supervision of the Behavioral Health Supervisor and or his/her designeethe Licensed Clinical Social Worker will provide concrete services and therapeutic interventions for patients and their family members. Treatment services will include psychosocial screenings, assessments, and case management functions. Provide direct service to patients and families. The social worker will participate in multidisciplinary case conferences, attend provider meetings, and perform behavioral health integration activities with the goal of primary care - mental health integration.PRINCIPAL DUTIES AND RESPONSIBILITIES: Provide intensive, integrated professional mental health services and treatment planning in a clinical and/or outpatient environment. Establish and/or maintain clinical practice guidelines and collaborative care links with the center's primary and complementary care systems. Work collaboratively with primary care providers to ensure continuums of care are maintained for primary care patients. Provision of professional leadership in the determination of appropriate therapeutic approaches and methods and planning and implementing outreach/education programs, workshops, and activities. Establish and maintain case files and related documents for clinical and legal purposes. Provide counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position; prepare treatment plans, discharge plans, and follow-up care programs; provide therapeutic crisis intervention and emergency services as required. Collect data about patients through interview, case history, psychological tests, and/or observational techniques; evaluate data to identify causes of problems and to determine proper therapeutic approach or referral to other specialists. Consult with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refer clients to appropriate service agencies as required. Provide and/or arrange for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems. Refer clients to appropriate social service agencies for financial assistance and other required services. Follow up to determine reliability of treatment used; change method and degree of therapy when indicated. Establish and maintain case files, referrals, and other related documents for the treatment of clients; prepare related documentation for civil commitment hearings or other legal proceedings; keep abreast of patient data to ensure appropriate treatment and care are delivered. Consult with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities. Plan and facilitate community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools. As appropriate to the position, perform various administrative functions such as monitoring budgets and preparing reports and correspondence; may participate in a variety of research projects to develop service plans, and may propose changes to program policies and procedures. May provide formal teaching, consultation, and in-service training to relevant professionals in immediate and proper handling of and/or referral of a variety of matters. Perform miscellaneous job-related duties as assigned.Requirements: LCSW licensure and certification, must be Spanish/English bilingual with 5 to 8 years of experience in clinical social work practice LCSW must have extensive knowledge of a multiple clinical modalities, theory and principles of human behavior, social work theory and practice Experience in a primary care and mental health setting is preferred Previous experience with substance abuse population is desired Child and Adolescent experience is desired Position requires travel to network of collaborative agencies and facilities as needed and 40 hour work week hours of operation Knowledge of clinical operations and procedures. Skill in preparing and maintaining patient records. Knowledge of community mental health resources, community support services and funding agencies. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Ability to evaluate the progress of therapeutic programs and to make individual modifications. Knowledge of crisis intervention techniques. Knowledge of the principles and methods of social and vocational rehabilitation as they relate to mental health clinical practices. Knowledge of legal and ethical issues related to patients' rights.
