Register patients for clinic appointment.2. Conducts interview of patient to obtain routine and appropriate health history information, documents pertinent information on patient's medical record on-line module1. Review Advance Directives Acknowledgment Form with patient.4. Pre-Visit Planning Accesses results of Lab work, procedures, consults, x-rays and schedule tests as needed. Inform patients to bring all medications to visit, including vitamins and herbal supplements being taken. Inform patients to also bring in their glucose, blood pressure or weight monitoring records and peak flow meters as appropriate.5. Performs visionhearing testing.6. Take EKG's on patients.7. Perform phlebotomy duty.8. Run controls for Glucometer and Waived Testing Kits.9. Performs Waived Testing (Point of Care).10. Assists with setting up exam room.11. Calls and directs patients to examination rooms.12. Takes and records vital signs and documents in electronic medical record (EMR).13. Reports out of rangeabnormal screeningtesting done to HNdesignee immediately.14. Prepares patients for examination.15. Assists patients, MD and nurse.16. Schedule follow up appointments for routine patient visits.17. Checks and secures stationary and other supplies as directed.18. Check temperature of medication, specimen and nourishment refrigerators twice daily, documents in temperature logs and reports findings to HNdesignee19.Maintain instruments and equipment and transport them when necessary.20. Transportsescorts patients when assigned.22. Replenish treatmentexam rooms.26. Arrives and leaves patient area at designated time.27. Completes assignment in allotted time.28. High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization; and2. Successful completion of training courses including the performance of phlebotomy andelectrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program; and3.
