We are currently searching for a Case Manager for a Community Services Nonprofit. The position is full-time in-person temporary to permanent position with an anticipated salary of $55,000. ORGANIZATION TYPE: Community Services NonprofitLOCATION: New York, NY (Midtown West) POSITION TITLE: Residential Case Manager SCHEDULE: Full-Time | 8AM-4PM or 9AM-5PM DURATION: Temp to Perm HOURLY RATE: $20.00-$24.00 Is This Your Dream Job? As the Residential Case Manager, you will support older New Yorkers by providing case management and rehabilitative services to residents in a supportive housing program. Your role is essential in preventing homelessness and hospital recidivism while fostering self-sufficiency, improving quality of life, and connecting residents with vital community resources.Our Ideal Candidate:BA in a related social service field with at least two years of experience.Has experience working with seniors, formerly homeless individuals, or those with mental health diagnoses.Demonstrates strong case management skills, including care coordination and advocacy.Possesses excellent written, oral, and interpersonal communication skills.Is proficient in Microsoft Office and cloud-based applications; knowledge of AWARDS database is a plus.Thrives in both independent and team-oriented environments.Manages time effectively, stays organized, and works well under supervision in a fast-paced setting.Is bilingual (preferred but not required).How You Will Spend Your Day:Conducting needs assessments and providing case management services to residents.Connecting residents with income resources, public benefits, medical care, and social services.Offering crisis intervention, de-escalation, and life skills support.Advocating for residents and coordinating care with multidisciplinary teams and service providers.Accompanying residents to appointments and hospital visits as needed.Managing resident safety, conducting wellness checks, and ensuring engagement in services.Maintaining thorough documentation and compliance with reporting requirements.Collaborating with internal teams and external partners to enhance resident well-being.Assisting with community programs, including meal deliveries when necessary.Participating in staff meetings, training, and professional development opportunities.We'd love to hear from you.If this sounds like the job for you, we would love to help make that happen.
