Catholic Charities provides comprehensive care coordination and case management services to individuals living with serious mental illness, complex medical needs and substance use needs. Our Targeted Case Management program provides comprehensive, community-based services for adults living with serious mental illness andor substance usedisorders, ensuring clients have access to uninterrupted and coordinated behavioral and physical health services while addressing the social determinants of health that impact daily living. The TCM Supervisor will provide oversight and direction to the Targeted Case Managers delivering care directly to clients. The Targeted Case Management program with a part of the larger Certified Community Behavioral Health Clinic (CCBHC) programs, including those certified by OMH and OASAS as well as those funded by SAMHSA.RESPONSIBILITIES:The Targeted Case Management Team Supervisor will support the program's efforts for client-centered and recovery-oriented care and plays a vital role inensuring the TCM program meets health outcomes for clients, such as reducing hospitalizations and emergency department visits, while increasing attendance andengagement with behavioral health and medical outpatient services. The TCM program will focus on providing whole-person supports for individuals living withco-occurring mental health, substance use disorder, andor health-related social needs, particularly during times of transitions such as from a residential treatment,hospital emergency department, or psychiatric hospitalization. Targeted Case Management Team Supervisor is responsible for ensuring direct service staff addressall social determinants of health such as medical needs, behavioral health and substance use needs, housing, food insecurity, educationemployment. This is donethrough partnerships and integrated work within the CCBHC and with other community-based providers, hospitals, and other systems of care to ensure clientshave access to a comprehensive healthcare network. The role will also require on call coverage and a willingness to be flexible.Targeted Case Management Team Supervisor is also responsible for ensuring that team's service delivery activities are person- and family-centered centered,culturally competent and linguistically capable. The Supervisor must also ensure complete documentation through electronic health record keeping; and have theability to interface with various websites as required by the CCBHC. Targeted Case Management Team Supervisor will ensure team's efforts to achieve highquality,timely, safe, effective and equitable referral or transition of CCBHC clients.Targeted Case Management Team Supervisor will direct and oversee team's efforts for client-centered care first and foremost while maintaining the additionalgoal of reducing avoidable health care costs, specifically preventable hospital admissionsreadmissions and avoidable emergency room visits, and improvingclient's outcomes by addressing primary medical, specialist and behavioral health care through contractual arrangements with appropriate service providers ofcomprehensive, integrated services. The Supervisor will also ensure the program is meeting required deliverables of the CCBHC and that the services follow allCCBHC standards and regulations.The TCM Supervisor is required to utilize technology and various web-based platforms for documenting progress notes and daily work activities. Ability to useequipment such as iPhones, tablets, Surface Pros and easily navigate various technology platforms and reporting systems is a requirement.SPECIFICATIONS FOR EDUCATIONCERTIFICATIONSLICENSESA Master's Degree in Social Work, Nursing, Public Health or other related social service or health profession.A NYS RN, NP, LMSWLCSW CASAC, License or a Licensed Psychologist preferred.SPECIFICATIONS FOR EXPERIENCE AND TRAINING•At least 1 year of experience in the management of a team, with progressively increasing responsibilities.•At least 2 years' experience working within the Human Services field, and experience working with individuals living with mental illness,substance use and other complex chronic health conditions.•Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.•Excellent computer skills are necessary.•The position requires a combination of skills in the areas of crisis assessment, clinical intervention, time management, organizational andmanagement skills.•Knowledge of the community medical resources and their financial requirements.•Good oral and written communication skills.•Knowledge of mental illness, mental health legislation and regulations, and health conditions.•Ability to manage multiple projects and ask for help when needed.•Ability to participate on a team to accomplish tasks.•Serves as a role model to staff and stakeholders.•Refined knowledge of techniques for identifying and preventing crises, including crisis management techniques.•Ability to work with all stakeholders: members, network, families, and government staff in a caring and respectful manner, and with dueunderstanding of and consideration for cultural differences.•Competency in written, interpersonal, verbal and computational skills to present and document records in accordance with program standards.We offer competitive Salary and excellent benefits including: generous time off, Medical, Dental, Vision, Retirement Savings with Agency Match,Transit, life insurance and other additional voluntary benefits.
