Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Work Shifts8:00 A.M - 4:00 P.MJob DescriptionPurpose:Under supervision of the Social Worker Supervisor Level V andor Associate Director of SUD Services and as a member of the facility leadership has the responsibility for clinical supervision of clinical staff involved in the treatment and care of substance using patients, the mentally ill andor emotionally disturbed, adolescent, adult and geriatric patients. Provides direct clinical social work and concrete services. Performs related administrative and performance improvement and clinical duties as needed.Specific Duties and Responsibilities:The Social Worker IV is charged with and responsible for:1. Participates in Performance Assessment and Improvement activities including collection of data, preparation and presentation of required reports as assigned.2. Provides daily unit coordination duties.3. Provides clinical supervision for and evaluation of assigned employees (including peers, interns and students).4. Manages a high-risk patient case load and consults with Interdisciplinary team both SUD and departmental (PIPMH OPHD, ED Leads and CATCH team on difficultcomplicated cases.5. Coordinates care with facility Medical Director, program MD Internist and Ancillary Withdrawal PANP6. Orients and trains newly hired staff; including staff of other disciplines.7. Manages high-risk patients and completes patient suicide screening tools (CSSR), psychosocial assessments and preparation of Patient Safety Plans.8. Supervises and tracks discharge planning; the provision of concrete clinical services, aftercare placement needs, and directly intervenes in managing high-risk SUD patients.9. Participates in the formulation of policies and procedures.10. Participates in program PA&I committee.11. Reviews and directs the preparation of reports and case histories; including chart reviews and clinical supervision of subordinate staff.12. As Internship Coordinator (SIFIE certificate holder), provide recruitment and then clinical supervision to two 2nd year Master's Level Social Work andor Mental Health Counseling students; coordinate with their prospective colleges. Responsible for school required evaluation of students' performance. As a SIFI trained supervisor supervises social work interns as needed.13. May lead andor participate in special committees and work groups, on & off site.14. As primary counselor provides clinical social work services individual, group treatment and discharge planning for assigned patients. Prepares psychosocial assessment, treatment and discharge plans. Provides crisis interventions where needed.15. Maintains relationships and contacts with outside agencies, including referrals to treatment and residential providers.16. Provides Overdose Education and Counseling.17. Attends and participates in hospital and off-site meetings, in-service education and community educational events.18. Provides supervisor on duty coverage for evenings and Saturdays when clinic is open.19. Directs clinical Social Work counseling services to a caseload of patients. Responsible for full psychosocial assessments, CSSR and Safety plans, treatment planning, chart notes, treatment plan updates, discharge summaries. In the course of treatment, responsible for full discharge planning, including referrals to Case CoordinationHealth Homes, ACT, AOT and other relevant stakeholders.20. Performs other needed activities and responsibilities at the discretion of program management.Minimum Qualifications1.
